Barcode Labels - Printing Item Name

We are using a thermal printer to make individual labels for our inventory items. Right now the barcode and barcode # is showing up on the label.

Can anyone tell me how to make the item name show up as well?
I found the custom pdf and it's all on there, but what form (?) is using this pdf - I can't find anything that has a 'Printing Fields' tab that's related to Inventory Items, Labels, etc...

Can someone help me!?!? Thanks!
