Duplicate SO created from a single PO

Hi

We are having some trouble with duplicate Sales Order. One of our user is experiencing this problem. Its not happening all the time but there were couple of example.

Problem is, when she tried to create a Sales Order from 1 Purchase order. on backend it auto creates a duplicate sales order at the same time or in 1 min difference. She is not aware of the duplicate sales order, until customer call and told that same order was sent twice. Some examples is attached:

Does anyone else face this type of problem? Can anyone help me with this? if anyone can help with creating a saved search which shows duplicate SO will also help. thank you for all your help
