Embed Online Form without IFrame

I'm trying to embed an online customer form. I saw a previously posted question with this same issue but the solution was not posted. Also I tried this article from Suite Answers:

Embed an Online Form in Web Site Page Without the Use of an I-framehttps://netsuite.custhelp.com/app/answers/detail/a_id/38299/kw/embed%20online%20form.

The issue I'm having with this solution is the form is not sending. Looking in the console there is 20+ JavaScript errors (see img).

Following the above article I:

  1. Previewed the Online form from the Publishable Form URL
  2. Viewed Page Source
  3. Copy/Pasted HTML/CSS/JavaScript from the form to my website
