Custom form fields are not doing a column break as marked in the form settings

Hello! We have a custom form that I'm trying to create column breaks by checking the column break checkbox. No matter which way I have tried it will not create column breaks in the form. It is still putting everything in ONE column. I have pictures below showing the form settings and what the output on the screen is. Am I missing something?
