List Price not showing correctly on forms

We have our List Price print out on our invoices because of requests from customers. It was just brought to my attention that it was not accurate on one invoice. The amount listed was nowhere near correct for the list price as it shows in our price level matrix. I did some digging and realized that the list price displayed is exactly 1/4 of the real list price. Then I realized the item for which the incorrect list was showing is purchased in eaches but stocked and sold in boxes of 4. This got me to
