Script not running on pageInit (new record)

Hello,

I was hoping someone could help me figure out what it is I'm missing :S

I have the following ClientScript, very barebones:

I have this deployed on Item Receipts:

My goal is to run a script when a user receives a purchase order and it creates a new item receipt. I would like the script to trigger once the new item receipt page finishes loading. However, I can't even get the log.debug to work, the script isn't triggering and I can't figure out what it is I'm missing :S

Any help is greatly appreciated!
