How can I customize Invoice Presentation Template (IPT)?

Does anyone have an idea or a workaround for the following in the Invoice Presentation Template Suite App:

Adjust the column or field size of the details (charges) page. Would like to increase the size of the Item field.

Edit the Charge type title

Generate Total or sub-total for the Quantity field. Would like to have a subtotal for Quantity just like Amount.
