How to make my Workflow that sets a Checkbox field work?

Good Day Everyone,

I have a workflow, which I will attach photos of, to check the Exclude Commissions box on our Return Authorizations if the Order Type = Replacement Order. The workflow will not execute now, and when it did (4 Times) the state was considered, but deemed false, so it didn't check the box. I don't know where I went wrong. Please help!
