How to use preferred Custom US Voucher Check?

I have created a new Voucher check form for printing checks. The preview looks good and have it set to be preferred. However, when I go to print a check from with either the "Write Checks" list or the "Print Forms and Checks" list (with Voucher being selected as the check type), the custom form is not being used.

I have checked several times to make sure the correct form is checked as preferred. Is there another option somewhere that I need to set?
