Need transfer order packing slip with transfer price listed for customs

We have a location in Canada, and our main warehouse in the US. We are entering transfer orders between the two.

We have the "use item cost as transfer cost" option turned on in accounting preferences. We also have the box checked on the transfer orders. There are 2 problems:

1) the transfer price is still always 0, and I'm not sure why.

2) We need a document that has the items and transfer prices for customs purposes. How do we create this?
