Custom Item Fields not Appearing and what is this 'Item Options' box?

Good morning, need a little help as I have two custom fields that aren't appearing on item forms, though I'm sure they were working at some point. I also have a box that's just a big error on my custom subtab and I'm not certain what that's from.

Here is the item form. In the customs tab I'm expecting to see two custom fields, the fields are still there as their information is being used elsewhere in the system and can be updated via csv upload, they're just hidden. Moving the fields onto other subtabs does not make them visible either.
