Create Inventory Count screen

In the Create Inventory Count" screen, how can I make the "count start date" field to default to a specific date, and make it "Disabled"? (see picture for reference)

I have tried workflow and script, however, since there is nothing when this screen shows up, they do not make any effect. I cannot even find the screen in the "Customizations - Forms - Entry Forms" list. So, I guess what I need is help understanding how to get to this screen, before it is displayed.

Any help would be greatly appreciated.
