How to find template that matches packing slip that prints?

I need to make a change to the packing slip. However the slip that currently is printed from the item fulfillment record does not match any templates in NetSuite. The sales order form being used has no link to a packing slip (so defaulting to standard preferred). I looked at the standard advanced pdf form and the Transaction Custom PDF Layouts. None match what I'm seeing when I print the slip. This is what I see. Any thoughts or help will be greatly appreciated.
