fields refused on online customer forms

This is related to Online Customer Forms. I'm trying to save forms, but I'm getting these errors. The form just fails and erases everything I've done if I have these fields in the form. Why are these fields returning errors?

In particular, I want to use the File field to have the user upload a file. I want to upload 2 files, as well. Is that even possible or is one the max (actually I've heard 0 is the max for customer forms and only Cases can have files).

Error is below for, in order, including the unsubscribe field, file field, and web address field.
