Journal Entry CSV upload template error

I'm trying to create a journal entry upload template. It keeps coming back with the error "Invalid account reference key xx for subsidiary yy".

The subsidiary setup is such that the Holding Company is the parent and there are 2 subs

The chart of accounts is setup to include children.

Attached is a copy of the csv template. What am I doing wrong?
