Add Invoice in Bank Reconciliation

Hi All

Please help to advise why sometimes our client finished recon 1 transaction, then recon the other one, choose add invoice then choose the transaction and save changes, suddenly the transaction is gone or disappear and need to choose again? 

Already try use chrome, opera, firefox still same?
