Upload File (Bank) - CSV Import error: incorrect number of CSV columns in the header (Memo column)

I've tried removing commas, dashes, periods, and other characters; also tried keeping the char count under 60, but this error keeps coming back. Does anyone know what I'm doing wrong? Thanks.
