Custom form field on vendor record

Hi all

I am trying to create a search that will show me a list of all my vendors with a column displaying the custom form of each vendor record.

I want to delete old and unused custom forms, but I can't see which forms are in use right now.
I also tried to build a SQL query, but this field isn't revealed.
