When customer applies promo code, they receive an error "Invalid taxcode ..."?

When a customer applies a promo code while logged out of their account, they receive an error code "Invalid taxcode reference key -706 for shipaddresslist <NULL>."

The only way around this is if they log in or close their browser and then log in. Regardless, the promo code refuses to work so long as the customer is not logged in.

Why is this happening?

How can this be fixed prior to logging in?

What does a promo code have to do with taxes?

Edit: Also, the promo code does not work on the shopping cart page; it has to be on the checkout page.
