Cannot specify custom form fields in Sales Order

I would like to add multiple custom forms to a Sales Order and be able to specify the form when creating the Sales Order, but there are no custom form fields in the Custom Transaction Form.

How do I make the fields appear?
I want to be able to specify any form when creating a Sales Order.
