Standard Customer Payment PDF/HTML Template

hi

the "Standard Customer Payment PDF/HTML Template" is attached with "Standard Customer Receipt" form. when used the pdf is comming correctly.

when i use "Standard Customer Payment PDF/HTML Template" with any other custom form(other 3 different custom form) that is customized from "Standard Customer Receipt" form it is not displaying same PDF layout when used with "Standard Customer Receipt" form

this is strange behavior no clue found any where. what to do?
