View where the demand originated in supply plan (Search or Report)

I need some help.

I'm trying to hold Procurement accountable by creating a report I can export into Excel for on time ordering.

the supply plan doesn't show where the demand originated from. It just shows the quantity and the need by date.

Is it possible:

  1. to have the supply plan show the Work order/ Sales order per line item in the supply plan?
  2. Can a report be created that shows open/Closed supply plans with the requested order date, the purchase order created (if created), and the dates involved with both?
    1. I want to generate a report that basically looks something line below:
