Custom Transaction Body Field that Filters on Customer Contacts

We have a case where a user wants to add the Requested By on a Sales Order and have that contact shown on the Advanced PDF/HTML Template.

I have been working on this for a while now and everything that I have set up should show the contact from the customer, however it is not filtering correctly, in fact, it is not showing any 'contacts' at all but when adding a NEW contact via the custom field the new contact does show in the custom field and on the customer.

What I have done so far:

  1. Created custom transaction Body Field called Requested by Contact
