Item Fulfillment in Picked Stage Without Line Items

Hello Gurus,

Need some helps here.

This is one of the strangest things I've seen from NetSuite. We have an item fulfillment in picked stage but without any line item. Please see screenshot below:

It even shows G/L impact.

How could this happen? How can I identify the root cause? The system notes is very limited.
