Customize the Vendor Center Purchase Request form

We would like to customize the Purchase Request form that opens when a vendor with Vendor Center access clicks a PO. We would like to create a few custom fields such as Updated Ship Date and Memo, that the vendors can fill in.

I then want to create a workflow that send our PO team an email to tell them the vendor has entered information.
