Duplicated Planned Purchase Order

I tried to search for an existing topic, but I was not able to find it…
Let me describe my issue:
Let’s suppose we need item X, quantity 1, demand date = 2nd November 2025 and today is the 1st september)
let's suppose that mrp generates a planned purchase order 123, item X, quantity 1, lead time 60 days
Planner releases that planned purchase
noone convert it in purchase order
MRP run again, 1 week after, and a different planned purchase order 456 is generated, item X, quantity 1...
so at the end, we have 2 planned purchase orders, and in fact generates a duplication of requirements.
