Make Copy OSM Purchase Order returns error invalid customform reference key 62

Make Copy of existing OSM Purchase Order using a custom form is returning error message = invalid customform reference key 62

This is NOT a new custom form, new vendor, new assembly, new BOM or new procedure. We have used the Make Copy for this purpose for over a year and never had this error message

Any thoughts greatly appreciated
