Who has template(s) for creating New BOM Revisions?

We have tried using the attached files to create new BOM revisions, but have found that only the 1st revision is created accurately, and then all proceeding ones either fail, or they are seemingly successful but use components from other BOM Revisions. Attaching the error files as well, which we also find very unhelpful as they refer to BOMs not associated with the External IDs.

Any guidance or just sending a template you've had success with would be appreciated.
