Online form templates (HTML) available outside NetSuite (Generic Lead Form)

Hello!

I submit my question as a community 'newbie', please excuse any initial snafus and/or submission format errors…..I'm in a pinch so here goes (BTW, 15 yrs experience w/ Oracle Erp)

Where can I download / obtain a 'generic' HTML Template (Basic Lead fields)?

I can 'tweak' template…. upload to FIle Cabinet… and complete Online Form Template

We have (2) separate websites (subsidiaries in NetSuite) hosted on Wix; one of which is 'suspended' due overlimit spam/bot crawlers.

We have been on a 12 hr sprint to complete "Embedding Online Form in Website Page"
