Scheduled query not bring back and data

Hi, I have created a report to send a notification when a certain trigger has been hit but it is not bring back the data, I have created the test data so know there data is there to be brought back but for the life of me cannot figure out what I am doing wrong.
This is the result i get back.
I have also attached a copy of each 'tab' and how i have completed them.
