Capturing Reasons for a Canceled Req based on Pre-Defined Fields

We would like any suggestions/ideas/methods other companies have tried and been successful with on capturing reasons for a canceled req based on pre-defined fields. 

Has anyone else had the need to capture reasons for reqs being canceled and if so, how did you go about capturing that and reporting on it?
