Anyone having problems with courses completing even if post-test failed?

I have some learncenter courses that have an instance with a post-test assigned to it. That course will then either get assigned or be part of a learning plan. With the post-test, the idea is that the user won't get credit for completing the course until/unless they pass it with a specific score.

I just tested one a few weeks ago and it seemed fine. Now, when I assign the course - as soon as the user launches it, even if they shut the browser down without doing anything, the course and the assignment get marked complete.  This is creating a huge hiccup in some of my content I planned to roll-out companywide next week - help!
