Assessment Questions not Updating

I'm wondering why, when I update an assessment question - it does not change the question for the assessment and instance?

Am I missing something?

I have assessment question X.  I go and modify the question for X... when a learner goes to the instance that includes that assessment the changed question does not display, rather the original question.
