Error Message When Hiring a Candidate - pMetaObject Cannot be Null

Has anybody else run into this error message when you're trying to move a candidate from Offer / Accepted to Hired / External?  We've had a few users escalate this issue to us since we moved to Taleo 12 in November of last year.  In each of the examples that have been escalated I was able to hire the candidate without any issues.  After clearing IE junk (cache, temp net files, etc.) out and clearing both Java and Flash files, the user that escalated the issue was still unable to hire the candidate.  But in each cause I was able
