Report of multiple screening services

Hello,

I'm trying to build a dashboard of all the screening services for a candidate on a req.

For example:

Candidate Name	Start Date	Background Complete?	I9 Complete?	Tax Complete?
Candidate,Test	March 5, 2013	Yes	No	No

Has anyone else been succesful?

My merged dimensions don't seem to work out.
