Insight Report (Hires per Source) incorrectly showing hires as "Unknown"

Hi everyone, not sure if anyone else has seen this sort of problem. When we run this report, it shows 22% of our hires  with the source "unknown".

We have run every search we can think of to try to find every hire in the system, and make sure they all have a proper source category selected.

We can't figure out where those 'unknown' are coming from. Unknown is not even an option in our source drop down menu.

Any ideas why this is happening?  or how we can find those 22% to update their sources?
