tracking OJT training

I have employees who need to have their managers sign off on them completing a task.  I want to create some kind of graded event that the supervisor of an user will approve/grade.  This will in turn allow them to move on to the next task.   I need to be able to track completion by user and topic.  I would like to combine them in a learning plan if possible.  Can someone help?
