Audit Trail Report and Custom BI Report

I'm trying to find a report that will show me somewhat of an audit trail of who might be manually enrolling users into our learning plans. I believe I had seen a standard report manager of an audit report, but I'm not finding this; can anyone tell me if there is a standard report manager report and/or how I can create a report that will display the username, first name and last name of the individual(s) who may be enrolling users into a specific learning plan.

Secondly, I had created a BI custom report yesterday and for whatever reason, it's not showing in my view that it was even created so I was thinking that I may not have saved it....however, when I go to re-create the report using the same report name I receive a message that the report already exists but yet I'm not able to see this report....weird...anyone else experience this and/or can recommend how I can access/run a report that claims to be created but I'm now not able to see?
