Auto rejection not working on some cancelled/filled reqs

We have noticed on some reqs in our system that when filled/cancelled, the candidates are not getting rejected and the auto rejection letter is not sent. We raised a SR to support on this and awaiting response but we were wondering if anyone else has experienced this recently? Not all reqs have the issue but it is sporadic. anyone?
