Has anyone utilized a field within Learn to collect information for a learner?  I want to  allow a learner to be able to select an option from a drop-down field and it be saved to their profile.  I see a forms option in Learn, but unsure of how to set it up.  This would be added to a learning plan for them to complete.  Has anyone been able to do this?
