In search of a basic termination reoprt

I am looking to create a standard termination report for the HR teamContent
Our company is new to Oracle and we were not provided what I'd consider a basic termination report with our implementation. To make matters worse, my SME resigned near go-live and I am not at all familiar with reporting. But I have figured out some basics of how to manipulate/add fields ("subject areas"?) and add filters. If someone could provide a screen shot of your termination report that shows where I find the necessary fields I'd be most grateful! I need the basics- ID, name, term date, term action, term reason. I can add the rest. Thank you!
