Report for delegation using BPM worklist vacation rule screen

A report is required to show BPM Workflow information where a task has been assigned due to a vacation rule taking effect. We are trying to get the following information. 
1) Person Name of the delegator 
2) Person Name to whom the role is delegated 
3) From Date 
2) To Date
