Learning Plan Completions not recording properly?

Mapped Items (Courses) completed 11/18 and 11/19 not completing learning plans

Content

I have multiple instances of mapped items/courses which were completed on Sunday 11/18 and Monday 11/19 but which are not updating the learning plan as "completed." For example, there is one course called "3E SDS August 2017" which is the only item mapped to a learning plan called "3E SDS."

On 11/18 at 12:46 pm, User #1 completed the course with 86% correct; passing for completion is 80%. In Learn, the course is "Completed" while the learning plan is Incomplete.

On 11/18 at 6:00 pm, User #2 completed the same course with 86% correct; in Learn, both the course and the Learning Plan are completed.
