Receiving Remote Region Error when trying to make actions for an Action Plan

When I try to save and close an action I receive a remote error region. Please see steps and Error below:

1.) Manage Action Plan Action

2.) Create Action:
- Type: Task
- Visibility: Internal Only
- Category: None
- Duration: 5 days
- Description: xyz

3.)  Setup Status Mapping:
Attribute Mapping tab and Fill in the following fields:
- In the 'Status' column, select 'Completed' from the dropdown
- In the 'Attribute' column select 'Outcome' from the dropdown
- in the 'Operator' column select 'Is one of' in the dropdown
