Scheduled approve received error - At this time, it is not possible to respond to this request for approval: 19003414 -Internal Auditor

A new requisition was created and two approvers were scheduled to approve.  Approver one successfully approved the req., but when approver two attempted to approve via email link she received the error:

At this time, it is not possible to respond to this request for approval: 19003414 -Internal Auditor

To workaround, the recruiter approved on the managers behalf, but we are wondering what created this error.  Can anyone advise if they have seen this before and what would cause it?
