Personalizations Log. What is the best way to document personalizations/customizations?

Summary

we've done quiet a few personalizations in our environment and would like to keep a comprehensive list of all our personalizations.

We've used a spreadsheet that our implementation consultant had started to document personalizations, which was awesome.  Over the years it became overwhelming and not very user friendly.  I'm wondering what other companies do to document personalizations?
