MSS My Team Actions - Fields For Editting

Hi Guys,

I would like to seek your help, we have a requirement wherein we need to add a new action reason in MSS My team page.

And when a manager selects the action reason a new set of fields will be shown to them.

Lets say if the action reason is Contractual to Probationary. then the contract type field will be shown for the manager to fill in or we can set it to a default value.

Your help will be appreciated.
