Trigger an online form for an address change

We are looking for solutions to trigger an employees to fill out an online form when they change address

Has anyone found a good solution for triggering an online form (for the employee to complete) when an employee changes their address through ESS? The specific example is the PA Act 32 form when someone moves to Pennsylvania but the question is general for having an employee take an action based on some type of logic. We've reviewed the following, but want to put the question out to the broader community - 

Option 1: Embed a URL to the form into the address screen. Ideally the link comes up when the employee selects a particular state. Not sure if this is feasible since we'll only need the URL to show up when a particular state is selected.
