What is the best way to pull dff's that make up the configurable form within a checklist?

After the 19B release, I have created an off boarding checklist that contains a configurable form which we are using for our exit interview surveys. The purpose of putting the exit interview survey in HCM is to pull the information in an analysis/report along with other related data.

I realize there is a subject area for checklists (Workforce Management Checklist Real Time), however, it does not seem to pull the data from the configurable form. Can you confirm this? If this is the case, what table can I pull in via data model to obtain checklist and form info.
