Requestion information notification from CSP at the particular phase and state is not getting trigerred

Hi Team,

I have created a CSP with the configuration to send "Request Information" to the candidate but whenever I am moving the candidate to particular phase and state the candidate is not receiving any email. Does anyone has faced similar issue?

I am attaching the document with the screenshot of configuration.
